You can reach the Cerulean Coast and the Southern Shore region in Elden Ring Shadow of the Erdtree DLC via the Castle Front -> Ellac River Cave -> Ellac River Downstream Site of Grace by following the river downstream while sticking to the left side.
From the Ellac River Downstream Site of Grace, stick to the right side and follow the main path. Soon, you will see a Furnace Golem. Be prepared to dodge his attacks while sticking to the right side.
Right at the Golem’s location, turn right, and you will see blue flowers at the end of your view. Follow the path on the right, and you will arrive at the Cerulean Coast and the Southern Shore region.

How to reach Ellac River Cave in Elden Ring Shadow of the Erdtree
To reach Ellac River Cave in Elden Ring, teleport to the Castle Front Site of Grace, take a left and follow the main path until you find the road splitting into two.
Now, take a right towards the downhill camp and keep following the main path.
When you spot three trolls on your right, take the left path and stick to the left side of the route.
Soon, you will arrive at the poisonous swamp area. Stick to the left and keep moving through the graveyard-like area until you reach the cliff from where you can see the cave below.
Enter the cave, climb the rocky formation on the left, and keep moving forward. You will arrive at the Ellac River Cave Site of Grace.
How to reach Ellac River Downstream in Elden Ring Shadow of the Erdtree DLC
Starting from the Ellac River Cave Site of Grace, move straight and take a left towards the waterfall.
Jump on the middle rock and keep moving, then jump on the right one, move straight, and jump on the left rock and keep moving.
Once you spot the deathbird-like enemies, stick to the left side until you reach the end of the waterfall.
From there, take a right and drop down the cliff step by step, and you will reach the Ellac River Downstream Site of Grace.
Where to find Southern Shore region map fragment in Elden Ring
Assuming you have reached the the areas Site of Grace, stick to the left side from there and keep moving straight.
The giant bull-like structure on a delipidated building is your cue that you are heading in the right direction.
Keep moving upward the hill, and you will reach the Southern Shore map fragment location.
